Great questions.  I believe you have to start with LinkedIn, and your own website/blog to help establish yourself as an expert in your field and geographic region.  LinkedIn will help tremendously with the marketing.  LinkedIn can also function as your personal website, but it will not be as effective as your own personal website or blog.  

To find facilities, you can scour the papers for therapist job ads, but your focus should be on long term relationships.  If you develop relationships with facilities, and the people at those facilities, you will benefit.  That process can be slow and painful, so I&#039;d recommend doing that &quot;on the side&quot; until you can build up your business.
